- Appointments are available Monday-Friday 8:00 AM to 4:30 PM
(phone lines are open 8:30 AM until 5:00 PM) - Urgent evaluations will be worked in as soon as possible or referred directly to our surgical facilities for immediate care
- We request that patients complete a patient registration form and medical history form prior to evaluation
- To save time and expense, when you make your appointment, please notify us if any of the following apply:
- You have seen another physician about this problem
- You have received diagnostic evaluations such as X-Rays, CT Scans, MRI or other tests from another physician
- You have had previous orthopaedic surgery for this problem
- Please Note: We do not require a physician referral. However, it is your responsibility to check with your insurer to see if a referral is required to assure that your insurance plan will pay for the appointment.
Workers’ Compensation
- Our registered nurses are members of the NC Association of Insurance Rehabilitation Professionals
- We work with you, your employer, the insurance carrier and case manager
- Our staff will treat your on-the-job injury and determine your return-to-work status
